My name is Dr. Bhagyashree Katare and I am a professor at Purdue University. My team of researchers at Purdue University are conducting surveys of Indiana small business owners and employees to increase the understanding of the wellbeing and sustainability of small businesses. We want first-hand information and the perspective of small businesses. We are simply trying to capture the experience of being a small business owner and employee in Indiana.
We are looking for small rural businesses in Indiana to participate in this survey. The survey will take less than 5 min of your time. On completion of the survey, you will be emailed a $10 Amazon gift card — via the email addresses provided by you at the completion of the survey.

Participation in this survey is completely voluntary. The responses will be kept confidential and personal identification will not be revealed during the analysis and write-up of findings. The participation of small businesses in these interviews would be a valuable addition to our research and findings could lead to a greater public understanding of small businesses in Indiana. The study has been exempted by the Purdue University Institutional Review Board (IRB#2021-1145).

What is the purpose of this study?
The purpose of this study is to increase the understanding of wellbeing and sustainability in small rural businesses in Indiana.
What will I do if I choose to be in this study?
How long will I be in the study?
You will be in the study for the period you require to complete the survey, about 5 min.
What are the possible risks or discomforts?
There are no known risks, discomforts, or insolvencies that are greater than everyday life and that are associated with the research of this type. Breach of confidentiality is always a risk with data, but we will take precautions to minimize this risk as described in the confidentiality section.
Are there any potential benefits?
There are no direct or societal benefits to the participants in this study. However, our findings could lead to a greater public understanding of small businesses in Indiana.
What alternatives are available?
You can always choose not to participate in this research study.
Will I receive payment or other incentive?
On completion of the survey, you will be emailed a $10 Amazon gift card. Once you complete the survey, you will be directed to an anonymous link to enter your email address. The Amazon gift card will be emailed within 7 days of completing the survey.
Are there costs to me for participation?
There are no anticipated costs to participate in this research.
Will information about me and my participation be kept confidential?
All information about you and your participation will be kept confidential. Data in electronic form will be stored on a locked computer in PI’s office on campus, and this consent form will be stored in a locked cabinet in a locked room on campus. The PI is the only person with the key to this cabinet and room. Data will be stored for three years after the conclusion of the study, and it will be destroyed after that. Only researchers will have access to this information, however, the project’s research records may be reviewed by departments at Purdue University responsible for regulatory and research oversight.
What are my rights if I take part in this study?
Your participation in this study is voluntary. You may choose not to participate or, if you agree to participate, you can withdraw your participation at any time without penalty or loss of benefits to which you are otherwise entitled. You can also restrict us from accessing any of your data.
